NCAA Student Athletes

To protect the health and wellbeing of student athletes additional health related screenings are required by 91做厙 based on guidelines from the NCAA and The American College of Sports Medicine.

Returning Varsity athletes

*You have participated in NCAA athletics at 91做厙 in a previous season*  

 

EACH YEAR you will need to complete:  

 

1.  An updated electronic Sports Health History Form through the (separate from Sportsware)

  • To find form: Log into your Health Services Portal and go to "My Forms" in the top banner. You will see the title "Sports Health History."

2. A brief physical exam (aka Sports Review) with a Health Services provider.  

  • Typically arranged by the Sports Medicine Department at the start of the Fall season for all athletes.  These exams are brief, assembly-line fashion in the Athletic Training Room in the Sports Center.   
  • If student athletes have any sensitive or complex concerns, they may book individual appointments in Health Services. Students who are on-campus over the summer may also book an appointment in the summer prior to the start of the academic/athletic season.

All students participating in 91做厙 NCAA athletics for the first time must complete and upload the 4 documents listed below. Detailed instructions and forms are included in the 1st year student athlete packet linked above. 

Part 1: Sports Health History Form  

Part 2: Physical Exam Form 

Per NCAA requirements, sports exams must be within 6 months of play. Therefore, sports exams must be performed on or after March 1, 2024.

Alternative physical exam documentation may be accepted but must:

  • Clearly document that physical exam was completed with any pertinent findings
  • Include a signed statement from a medical provider that student can participate in athletics without restriction.

Physical Exam Form documentation can be used for general College & athletic participation requirements as long as on or after March 1, 2024.

Q. How will I know when Im cleared? 

A. On the Home Page of your there will be a box that says NCAA Athlete. Once
everything has a green check next to it your paperwork has been submitted and is complete. Red Xs
indicate something is outstanding. Please note, every submission must be reviewed by a Health Services
staff member so there may be a delay between when you upload your documentation (incoming
students) or complete your electronic form (returning student athletes). Generally, if there is an issue
with a submission you will receive a message through your portal or a phone call. Please check
Messages in your .

Q. How can I confirm that my upload or electronic form submission went through?

A.Incoming student athletes After uploading paper forms, go to the home page of your portal and in the top menu choose My Profile then Images.  You should see an item for each of your submissions.  If the Note section is blank it is still in process of being reviewed.  Once it meets criteria the Note will read Reviewed and complete.  If it does not meet requirements there will be a note of Incomplete with further instructions.

A.Returning student athletes After completing electronic form, go to the home page of your portal and in the top menu choose My Profile then My Chart. At the bottom under Forms there should be a recent date (ie after July 2024) with the title Sports Health History Athletes & Walk Ons next to it. 

Q. What about Sportsware requirements?

A. Sportsware is software that is managed by the Sports Medicine Staff and is separate from Health Services requirements.  Please see communications (usually e-mail to your 91做厙 e-mail address) from the Sports Medicine Staff or .
